This condition can lead to tracheal or esophageal compression, or manifest through life-threatening complications such as aortic rupture or dissection. This paper reports a case of a 66-year-old male patient admitted with dysphagia and chest heaviness. Chest MDCT confirmed the presence of Kommerell’s diverticulum arising from a right-sided aortic arch associated with an aberrant left subclavian artery. The patient underwent successful open surgical repair, involving thoracic aortic replacement with a vascular prosthesis and transposition of the subclavian arteries into the common carotid arteries. This approach is the optimal strategy to eliminate the risk of rupture and definitively relieve mediastinal compression.</p> Nguyễn Quốc Cường, Lê Đức Tín Copyright (c) 2026 https://creativecommons.org/licenses/by/4.0 https://jsde.nctu.edu.vn/jsde/article/view/868 Mon, 27 Apr 2026 00:00:00 +0700 A Case Report of Grade IV Pancreatic Trauma Managed by Roux-en-Y Pancreaticojejunostomy with Blumgart Anastomosis https://jsde.nctu.edu.vn/jsde/article/view/867 <p><em>This case report describes a 21-year-old male with polytrauma following a motorcycle collision with a stationary truck, presenting with severe abdominal pain, peritonitis signs, and red gastric aspirate. Abdominal CT revealed peritoneal fluid, and grade II injuries to the right kidney and spleen, but no clear pancreatic description. Diagnostic laparoscopy identified retroperitoneal hematoma around the pancreas and complete transection of the pancreas left to the superior mesenteric vein. The patient underwent open laparotomy with closure of the proximal pancreatic stump (preserving the main duct), debridement, and distal pancreaticojejunostomy via Roux-en-Y with Blumgart anastomosis. Concomitant injuries (spleen, kidney, liver) were managed conservatively. Postoperative course was uneventful with no pancreatic fistula or major complications; the patient recovered well and was discharged. Clarifying the relationship between long-term glycemic control, as reflected by glycated hemoglobin (HbA1c), and lipid components may provide additional information for the early assessment of metabolic disorders. This study aimed to determine the prevalence and patterns of dyslipidemia and to investigate the association between HbA1c and lipid parameters by sex in adults aged 35–75 years newly diagnosed with hyperglycemia. A cross-sectional descriptive study with analytical components was conducted in 472 patients at Kiên Giang General Hospital from July 2024 to June 2025. HbA1c, fasting plasma glucose, and lipid profiles were measured using standardized laboratory methods. Dyslipidemia was identified in 83.5% of participants. Among lipid components, elevated triglycerides were the most prevalent (64.4%), followed by increased low-density lipoprotein cholesterol (55.3%) and total cholesterol (50.0%), whereas reduced high-density lipoprotein cholesterol was less frequent. HbA1c showed a weak but statistically significant positive correlation with total cholesterol, triglycerides, and low-density lipoprotein cholesterol, while no significant association was observed with high-density lipoprotein cholesterol. These correlations were observed in men but were not statistically significant in women. In conclusion, dyslipidemia is highly prevalent among adults newly diagnosed with hyperglycemia. Nerve damage can develop early and may remain clinically silent for years. As it progresses, it may affect sensory, motor, and autonomic pathways, leading to impaired quality of life and a higher likelihood of foot ulceration, lower-limb amputation, and premature death. Recent guidance from the American Diabetes Association has increasingly highlighted the need for comprehensive metabolic optimization, concurrent control of modifiable risk factors, and individualized care plans to improve prevention and treatment outcomes. In this review, we summarize current The American Diabetes Association recommendations on diabetic neuropathy, covering key aspects of definition and classification, underlying mechanisms, clinical presentation, diagnostic and screening strategies, and treatment options. We also discuss how these recommendations align with, and may be adapted to, routine clinical practice in Vietnam. Among them, the nasal septal spur is an anatomical abnormality accounting for approximately 12.5% of septal deformities and may cause localized airway narrowing, leading to persistent nasal obstruction. A 25-year-old male patient with a history of allergic rhinitis presented with prolonged nasal obstruction, headache, and rhinorrhea. Nasal endoscopy revealed a right septal spur with pale mucosa, while paranasal sinus CT-scan demonstrated right-sided septal deviation accompanied by a spur without evidence of sinus disease. The patient underwent endoscopic septoplasty, resulting in near-complete symptomatic improvement.
